We are currently accepting new patients at all three of the Mid Devon Medical Practice Surgeries.

As well as permanent registrations, we can also register people if they are staying in the practice area on a temporary basis – either up to 15 days or over 15 days and up to 3 months. You will need to provide the name and full address of your own surgery.  

Please contact the surgery nearest to your home to confirm that you are within our practice boundary.

Alternatively, Registration Forms and Patient Questionnaires can be printed and completed before bringing them into the surgery together with your identification documents. These documents can be downloaded from the Documents section on this page.

If you are permanently registering with us you will be asked to complete the registration form and you will be offered an appointment to see a doctor or a nurse for a ‘New Patient Medical’, when details of previous illnesses, current medication etc. will be documented.

Reasonable Adjustment Flag

The Reasonable Adjustment Digital Flag is a visible marker on a patients record which identifies the changes that need to be made by health and care services to make services as easily accessible as possible. This digital flag will be used to record, share and review details of a reasonable adjustment.

Your named GP

Although patients are registered with the Practice not an individual GP, we provide a named GP for everyone:

Named GPs take responsibility for administrative tasks such as medical reports or updating care plans, but may not be the doctor that you are always able or choose to see.
